In 2020 Valley Health Plan will expand their service area into San Benito County. They will be offering off-exchange health plans in Aromas (95004), Hollister (95023, 95024), and San Juan Bautista (95045).

Valley Health Plan is only offered in Santa Clara County. They are available through Covered California and off-exchange.
Enrollment Application
Online Enrollment
VHP is using a version of their PDF paper application for online enrollments through the web application DocuSign for off-exchange enrollments. If you would like me to be attached to the application to assist with any enrollment questions or health plan issues during the year, you will need to include my name, email address, and later on the application my license number 0H12644. After you have filled in the broker and applicant names, click on the button that says Begin Signing. That will take you to the next page to fill in the actually application. The completed application will then be forwarded to all household members over 18 years for their signature. It’s also best to assign a Primary Care Physician on the application. 2015 Valley Health Plan (VHP) offers HMO individual and family plans only in Santa Clara County. They offer health plans through Covered California for tax credits or mirrored products off the exchange. The Pediatric Dental benefits (children’s dental insurance for members 18 years and younger) is provided by Liberty Dental. VHP also offers Native American and Native Alaskan health plans.
