Sutter Health Plus is expanding its service area to include 15 new ZIP codes in Santa Clara County effective Jan. 1, 2020. access to care for members in the South Bay who live or work in Santa Clara County, primarily around San Jose. Additionally, employers and consumers in the expanded service area will have a new way to access Sutter-affiliated providers.
The service area expansion includes the following 15 ZIP codes:
Coyote: 95013
Watsonville: 95076
Mount Hamilton: 95140
San Jose: 95111, 95119, 95120, 95121, 95123, 95127, 95132, 95135, 95136, 95138, 95139, 95148
Members will have access to the existing network: Palo Alto Medical Foundation and Brown & Toland Physicians.
Members enjoy convenient features through our integrated delivery system, such as access to Sutter Walk-In Care clinics and My Health Online video visits, as well as innovative partnerships such as TeraPractice for virtual-first primary care and Alto Pharmacy for free same-day prescription delivery.
