Sutter Health Planโs 2027 individual and family rates are better than I expected in many Northern California rating regions. The plans are especially worth considering if you want access to Sutter-affiliated doctors and hospitals, are comfortable with an HMO, and know you will not receive a premium subsidy through Covered California.
There is one important limitation: Sutter individual and family plans are sold only off exchange. They are not available through Covered California, so enrollees must pay the full monthly premium.
Where Sutter Health Plans Are Available?
Sutter HMO plans serve portions of Northern California, including the Sacramento region, Solano County, the Bay Area, the Peninsula, parts of the North Bay, and areas extending into the Sierra foothills and northern San Joaquin Valley.
Depending on the service area, participating physician organizations may include groups such as the Palo Alto Medical Foundation, Brown & Toland Physicians, Affinity Medical Group, Sutter Medical Group, Sutter Independent Physicians, and Sutter Gould Medical Foundation. The network also includes numerous Sutter-affiliated hospitals and selected partner facilities.

Always verify the current provider directory before enrolling. A doctor, medical group, or hospital may participate in one plan network but not another, even when the carrier advertises broad access to the Sutter system.

2027 Standard Benefit Designs
Sutter offers the standard benefit designs required for Californiaโs individual and family market. These designs make the cost-sharing structure broadly comparable across carriers at each metal level. Summary of benefits for each metal tier available for download below.
| Metal level | Maximum out of pocket | Medical deductible | Primary care visit |
| Bronze 60 | $11,650 | $5,800 | $60 |
| Silver 70 | $11,650 | $4,700 | $50 |
| Gold 80 | $9,600 | None | $40 |
| Platinum 90 | $5,500 | None | $20 |
The Silver 70 medical deductible generally applies to major services such as hospitalization. Many routine services instead use a fixed copayment or coinsurance and are not subject to that deductible. Gold and Platinum eliminate both the medical and pharmacy deductibles and offer lower copayments for many routine services.
HMO, EPO, or PPO: What Is the Difference?
For the rate comparison, I am selecting health plans that include Sutter providers in the network. Health plans with Sutter providers in network include Anthem Blue Cross EPO and Blue Shield PPO plans. Because of the limited service availability of Sutter HMO in some counties, your only option may be either the Anthem Blue Cross or Blue Shield PPO.
Sutter HMO
An HMO generally requires you to select a primary care physician and obtain referrals for specialist care. Except for emergencies and other limited situations, care must remain within the HMO network.
Anthem Blue Cross EPO
An exclusive provider organization allows members to self-refer to in-network specialists, but it generally provides no coverage for out-of-network care. It can be a useful alternative when you want Sutter physicians with greater referral flexibility.
Blue Shield PPO
A preferred provider organization also allows self-referral and may provide some out-of-network benefits after a high deductible. In practice, members should still try to remain in network. PPO premiums can be higher because the network may include additional systems such as UC Davis Health or UCSF Health.
How the 2027 Rates Compare by Region
The comparisons below are based on premiums for a 40-year-old. Because carriers use the same age-rating curve, the relative ranking will generally remain similar at other ages, although each applicant should obtain an exact quote.
| Region | 2027 rate takeaway |
| Region 2 โ Solano County | Sutter HMO is the lowest-cost option among the compared plans for consumers seeking Sutter doctors. |
| Region 3 โ Sacramento area | Sutter HMO is the lowest-cost Bronze, Silver, Gold, and Platinum option in the comparison. |
| San Francisco | Sutter leads on Bronze, Gold, and Platinum. Anthemโs off-exchange Silver rate may be similar to or lower than Sutter after accounting for the exchange pricing difference. |
| Region 6 โ Alameda County | Blue Shield PPO leads on Bronze; Anthem EPO and Blue Shield PPO are lower on Silver. Sutter becomes more competitive on Gold and Platinum. |
| Santa Clara County | Anthem EPO leads on Bronze and Gold; Anthem EPO and Blue Shield PPO are lower on Silver. Sutter is the lowest-cost Platinum option. |
| San Mateo County | Sutter HMO is the lowest-cost option across Bronze, Silver, Gold, and Platinum in the comparison. |
| Region 9 โ Santa Cruz County | Sutter HMO offers the better rates across the compared metal tiers. |
One pricing detail matters when comparing Silver plans: Covered Californiaโs exchange Silver premiums are approximately 5 percent higher than equivalent off-exchange Silver premiums in this comparison. Bronze, Gold, and Platinum premiums do not have that same exchange adjustment.
| For consumers committed to the Sutter system, the 2027 HMO premiums are competitive across much of Northern California. |
Who Should Consider a Sutter HMO?
A Sutter HMO may be a strong fit if all of the following are true:
- You prefer Sutter-affiliated physicians and hospitals.
- You are comfortable coordinating care through a primary care physician and HMO medical group.
- You do not expect to qualify for a Covered California premium subsidy.
- You have confirmed that your doctors, medical group, hospitals, prescriptions, and routine services are covered.
If you might qualify for a subsidy, compare your net Covered California premium against the full-price Sutter premium before making a decision. A lower sticker price off exchange does not necessarily produce a lower final cost.
Bottom Line
Sutterโs 2027 individual and family plans are competitively priced in much of Northern California, particularly in the Sacramento area, Solano County, San Mateo County, and Santa Cruz County. The picture is more mixed in Alameda and Santa Clara counties, where Anthem EPO and Blue Shield PPO options may lead at certain metal levels.
Price is only one part of the decision. Compare the plan type, referral rules, prescription coverage, expected out-of-pocket costs, and the exact provider network before enrolling.
